AmeriHealth Mercy's PerforMEDSM Recognized For Innovation In Health Care Management
July 5, 2006 - Philadelphia (PA) - PerforMEDSM, AmeriHealth Mercy's care management program, was recently recognized as one of the top six finalists for the 2006 Thomson Medstat Innovator Award. This award honors organizations that are forging solutions to the quality and business challenges facing the nation's health care system.
PerforMEDSM was recognized for its proactive approach to case management, which uses prospective risk scores to identify high-risk members and manage their chronic conditions. The PerforMEDSM care management model, used at Keystone Mercy Health Plan, incorporates the best practices in intensive case management, disease management, and behavioral health coordination. PerforMEDSM generates an annual net savings of millions of dollars for a relatively small population of high-risk members with high medical costs.
