AmeriHealth Mercy's PerforMEDSM to Present Intensive Case Management Strategies for High-Risk, High-Cost Members
at 11th Annual Health Management Congress
September 6, 2006 - Philadelphia (PA) - Karen Michael,
RN, MSN, MBA, Vice President, Clinical Services, PerforMEDSM,
will present on Management of High-Risk and High-Cost Chronically
Ill Members through Intensive Case Management, at the 11th Annual
Health Management Congress in Las Vegas, on Thursday, September
14th from 2:20 pm -3:00 pm. PerforMEDSM, AmeriHealth Mercy's care
coordination product, provides an intensive case management
approach aimed at health plan members who are at risk for extensive
and long-term medical needs.
Traditional disease management is not fully effective in managing
high-risk, high-cost members with multiple, advanced chronic
care conditions, particularly when conditions are complicated
by psychosocial and economic issues. Through PerforMEDSM, AmeriHealth
Mercy has implemented a comprehensive program of care management
that incorporates the best practices in case management, disease
management and behavioral health coordination. In this session,
Michael will discuss:
- Strategies to identify high-risk members upon enrollment
- Implementing predictive modeling to identify and plan for
programmatic changes
- How this Best Practice approach is reducing costs while
improving quality outcomes and member satisfaction
With more than 20 years of experience exclusively serving the
Medicaid population, the AmeriHealth Mercy Family of Companies and its affiliates comprise

States, touching the lives of more than 2 million members in
