AmeriHealth Mercy to Present at the 2008 AHIP Medicaid Conference

- AmeriHealth Mercy's PerforMEDsm care management program will be featured during a breakfast session at the 2008 AHIP Medicaid Conference, being held in Washington, D.C. from September 26-27, 2007.

Lynne Harsha, Associate Vice President, PerforMED, will present Design and Development of Customized and Unique Care Management Approaches for Complex Medicaid Populations on Wednesday, September 24th from 7:45 am - 8:45 am. This presentation will focus on the issues that are unique to the Medicaid membership, and how the best approaches are determined by using predictive modeling and tailored clinical assessments. Ms. Harsha will discuss a care management (disease and case management) approach which is developed in coordination with the local health care community (primary care providers and medical home) and public stakeholders.

She will also discuss the AmeriHealth Mercy care management product, PerforMED. The PerforMED care management model incorporates the best practices in intensive case management, disease management and care coordination.
